Prosthesis Guarantee

Citydent Dental Prosthesis Warranty Form

This warranty program includes Citydent warranty terms for prostheses with metal-ceramic crowns, zirconium-based ceramic crowns, monolithic zirconium, multilayer zirconium, veneers and all-ceramic crowns. This warranty program begins from the day the permanent prosthesis is fitted.

1. Warranty Coverage

a. Breakage of the Prosthesis Other than External Causes 5-Year Warranty

Repairs and necessary replacements within the first 5 years after the prosthesis is fitted are guaranteed free of charge.

b. Chipping / Breaking of the Porcelain on the Prosthesis 5-Year Warranty

If the prosthesis is removable, it will be repaired; if not, it will be replaced free of charge within the 5-year warranty period.

c. Sensitivity to Hot and Cold in the Prosthesis Area 6-Month Warranty

The prosthesis will be removed, the relevant tooth treated, and the prosthesis re-fitted. If the prosthesis cannot be removed, it will be replaced free of charge. The cost of any additional dental treatments needed beyond the prosthesis is not covered by the warranty.

d. Pain in the Prosthesis Area 6-Month Warranty

Within the first 6 months after the prosthesis is made, it will be removed, the relevant tooth treated (at the patient's expense), and the prosthesis re-fitted. If the prosthesis cannot be removed, it will be replaced free of charge. Between the 7th month and the 5th year, the prosthesis will be removed, the relevant tooth treated (at the patient's expense), and the prosthesis re-fitted. If it cannot be removed and needs to be replaced, a new prosthesis can be made at 60% of the current treatment price upon patient request.

e. Food Particles Getting Under the Prosthesis 6-Month Warranty

Filling under the prosthesis body and, if necessary, remaking the prosthesis are covered free of charge within the first 6 months after the prosthesis is made. Between the 7th month and the 5th year after the prosthesis is made, filling under the prosthesis or, if the patient requests, remaking the prosthesis can be done by paying 60% of the current treatment prices.

f. Issues with the Aesthetic Appearance and Color of the Prosthesis

The warranty for remaking the prosthesis or changing tooth shapes is valid only during the temporary cementation stage. Requests after the permanent cementation are not covered. For aesthetic expectations, within the first 3 months, the prosthesis can be remade at 60% of the current treatment price. After the first three months, requests due to aesthetic expectations are not covered by the warranty.

g. Prosthesis Being Too High or Too Low 6-Month Warranty

Repair and necessary replacement within the first six months are covered. Low height criteria are defined as the inability to detect contact points on all teeth when the patient bites an articulation paper.

h. Inability to Chew or Grind Certain Foods with Prosthesis Teeth 6-Month Warranty

If contact points are not formed when the patient bites an articulation paper within the first six months, repair and necessary replacement are covered by the warranty.

i. Fit Issues of the Prosthesis in the Mouth 6-Month Warranty

Problems with the fit of the prosthesis in the mouth within the first 6 months are covered by the warranty. Adjustments and, if necessary, remaking the prosthesis to ensure a proper fit will be done free of charge during this period.

j. Speech Difficulties Due to the Prosthesis 3-Month Warranty

If speech difficulties occur within the first 3 months after the prosthesis is fitted, necessary adjustments are covered by the warranty.


2. Exclusions from Warranty Coverage

  • a.Damage to the mouth and teeth resulting from falls, traffic accidents, etc.
  • b.Applying unusual forces to the prosthesis. Examples: breaking pistachio shells, opening soda caps, biting wire, etc.
  • c.Issues caused by conditions outside the scope of the treatment or from another ailment or treatment.
  • d.Spacing that may occur between partial prostheses or between the prosthesis and natural teeth due to gum disease.
  • e.Problems with the prosthesis and teeth caused by bone loss in the areas where the teeth are located within the prosthesis due to gum disease.
  • f.Problems related to the health and aesthetics of teeth caused by the formation of a gap between the prosthesis margin and gum due to gum recession.
  • g.Issues arising from dental and gum health problems due to neglecting oral hygiene.
  • h.No intervention should be made to the prostheses by another clinic without informing Citydent for any reason. (If the prosthesis comes off due to the adhesive failing, having it re-cemented at another place does not void the warranty.)

Other Provisions

a. Treatments Outside Citydent: Citydent is not responsible for prosthesis problems arising from dental treatments performed outside of Citydent. Citydent is not liable for the costs incurred from prosthesis cementation, repair, or replacement performed elsewhere for prostheses made at Citydent. No refunds will be made to the patient in these cases.

b. Multiple Repairs or Remakes of the Prosthesis: Prostheses can be repaired or remade multiple times by Citydent within the warranty periods. The warranty period for a remade prosthesis does not restart; the original warranty period continues to apply.

If patients residing abroad return to Citydent for re-treatment purposes within the first 6 months after the initial treatment due to issues covered by the warranty program, Citydent will cover a portion of the flight and accommodation expenses, specifically €250, as a one-time offer. This applies to prosthetic dental treatments involving 10 or more units. A physician's examination is required to determine if the issue falls within the warranty coverage for payment to be made.

Implant Guarantee

Implant Waiting Period Warranty Conditions

The implant waiting period refers to the time awaited for the implant to osseointegrate (bond with the bone) and become stable after being surgically placed. The durations of this waiting period under warranty are specified in Appendix 1. Treatments covered by the warranty are stamped and indicated in the additional treatment plan.

Full Warranty

Conditions:

  1. The patient must visit Citydent for the next stage of the treatment within the dates specified in Appendix 1.
  2. The patient must follow the instructions specified in Appendices 2, 3, 4, and 5.

Scope:

  • Includes the removal of the implant if necessary and its replacement with an implant of the same brand or an equivalent brand free of charge.
  • If re-implantation is not possible, the cost of the relevant implant will be refunded to the patient.
  • Includes the remaking of temporary prosthetic teeth on the implant during the defined waiting period if remaking is required.

Partial Warranty

Conditions:

  1. Valid for 1 year from the last date specified in Appendix 1.
  2. Must follow the instructions in item 5.a of Appendix 5.

Scope:

  • The replacement of the implant with a new implant is free, but the removal of a failed implant and the placement of a new implant is charged. This fee is half of the current amount after deducting the previous discount rate given to the patient from the list price of the implant treatment.
  • The full cost of materials such as grafts and membranes needed in certain situations will be charged to the patient.
  • If remaking of temporary teeth on the implant is necessary, the new temporary teeth will be charged.

Additional Provisions and Conditions

Appendix 1 — Next Stage Durations

Earliest date for the next stage: as specified in individual treatment plan

Starting the second stage of treatment within the specified times above is crucial for the success of the treatment. Otherwise, Citydent is not responsible for any consequences that may arise.

Appendix 2 — Oral and Dental Care

  • a)The patient fully complies with the dentist's recommendations regarding hygiene around the implant and practises appropriate oral care accordingly.
  • b)The patient uses a toothbrush and dental floss as often as recommended by the dentist. They learn to use these tools correctly and take care not to damage the implant area.
  • c)The patient should not smoke during the waiting period.
  • d)If the patient encounters any problems with their oral and dental health, they must inform Citydent quickly and follow the instructions of their dentist.

Appendix 3 — Dietary Regimen

  • a)First 45 days: Considering that your temporary prosthesis has not fully integrated with the implants and jawbone during this period, only soft foods should be consumed. Medium-hard and hard foods should be avoided.
  • b)Between 45 days and the last date specified in Appendix 1: Soft and medium-hard foods should be consumed at this stage. Hard foods should be avoided.

Appendix 4 — Temporary Prosthesis Instructions

  • a)You can comfortably continue your daily activities while using the prosthesis known as a fixed temporary tooth. However, remember that the durability of this prosthesis is limited, it can easily break, and it may damage your teeth. Making permanent prosthesis within the durations specified in Appendix 1 is crucial for your oral and dental health.
  • b)In cases of loosening, breaking, cracking of the temporary prosthesis (temporary teeth), or discomfort or changes in the temporary tooth area, Citydent should be informed immediately, and you should act according to the instructions of your dentist at Citydent.

Appendix 5 — Procedures Performed Elsewhere

  • a)If the patient plans to have dental procedures done elsewhere during the waiting period, they must inform and obtain approval from their dentist at Citydent beforehand. Otherwise, this warranty program will be invalidated.
  • b)The Citydent warranty program does not cover procedures done at different examinations or clinics. If the patient has procedures done elsewhere on implants or temporary teeth, Citydent will not make any reimbursement to the patient.

Appendix 6 — Exclusions from Warranty Coverage

  • a)Implant loss due to damage to the jawbone, teeth, or prosthesis caused by significant forces such as falls, traffic accidents, etc.
  • b)Implant loss due to gum, jaw, and dental problems that may arise after implant placement due to health issues related to the implant surgery.
  • c)The patient's failure to pay the agreed and approved payment amount for the implant treatment.

Implant & Fixed Prosthesis

Implant and Fixed Prosthesis Warranty Form

This warranty program begins on the day the patient's implant and overdenture treatment is completed.

Implant Warranty Conditions

Implants may fail to integrate successfully with the bone for various reasons or may experience problems over time. This is referred to as "implant loss," and the following warranties apply in such cases:

1. Removal of a Failed Implant

The failed implant will be removed free of charge during the first 10 years.

2. Placement of a New Implant

  • a.Within the first 5 years, the placement of a new implant and the standard implant placement procedure will be free of charge.
  • b.Between 5 and 10 years, 50% of the current implant fee will be charged for a new implant and the standard implant placement procedure.
  • c.After 10 years, no warranty will apply to the placement of a new implant.
  • d.Within the first year, if graft, membrane, and PRF procedures are required, a 50% fee will be charged.

3. If a New Implant Cannot Be Placed in Place of a Failed Implant

Half of the fee paid for an implant removed within the first 2 years will be refunded to the patient. No refunds will be made after the first 2 years. There are no refunds for prosthetics. Prosthetic repairs or replacements will be carried out free of charge.


Fixed Prosthesis on Implant — Warranty Conditions

  1. Breakage of the Prosthesis: Within the first 5 years after the denture is fitted, its repair and, if necessary, remaking are guaranteed.
  2. Chipping / Breaking of the Porcelain on the Fixed Prosthesis: Within the first 5 years after the fixed prosthesis is fitted, its repair and, if necessary, remaking are guaranteed.
  3. Loosening and Breaking of the Fixed Prosthesis and Implant Screw: Within the first 5 years after the fixed prosthesis and implant screw are fitted, any loosening or breaking will be covered by warranty, including screw tightening, replacement, or necessary repairs. During this period, any problems arising from the loosening or breaking of the fixed prosthesis or implant screw will be addressed free of charge. The patient may have the fixed prosthesis or implant screw tightened or problems solved at a different clinic, with Citydent's knowledge, but no refund will be claimed from Citydent for such procedures.
  4. Failure of the Implant: If the implant or implants are redone free of charge within the first 5 years under warranty conditions, the prosthetics on the implant will also be made free of charge. If the implant fails after the first 5 years, the current tariff will apply for the making of a new prosthesis.
  5. Adhesive Problems with the Fixed Prosthesis (applicable to cemented fixed prosthesis): Problems with the fixed prosthesis adhesive within the first year after fitting are covered by the warranty. During this period, repairs due to adhesive problems and, if necessary, remaking of the fixed prosthesis will be free of charge. At Citydent, cementing procedures are free for 5 years. The patient may have the fixed prosthesis cemented at another clinic with the knowledge of Citydent. Citydent will not reimburse the patient for cementing procedures performed elsewhere.
  6. Temperature Sensitivity in the Fixed Prosthesis Area: Within the first 6 months after the fixed prosthesis is fitted, its repair and, if necessary, remaking is guaranteed. Other treatments that may be required, such as fillings, root canals, and gum treatments, are not included in the warranty program.
  7. Food Trapping Under the Fixed Prosthesis: Within the first year, filling under the fixed prosthesis body and, if necessary, remaking is guaranteed. Between the first and fifth year, filling under the fixed prosthesis or remaking can be done for 60% of the current treatment price.
  8. Fit Issues with the Fixed Prosthesis in the Mouth: Problems related to the fit of the fixed prosthesis in the mouth within the first 6 months after fitting are covered by the warranty. During this period, adjustments to ensure the proper fit of the fixed prosthesis in the mouth and, if necessary, remaking will be free of charge.
  9. The Fixed Prosthesis Being Too High or Too Low: Within the first 6 months, the repair and, if necessary, remaking of the fixed prosthesis are guaranteed.
  10. Swallowing Difficulty Due to the Fixed Prosthesis: Within the first 3 months after the fixed prosthesis is fitted, any swallowing difficulties caused by the fixed prosthesis will be addressed under the warranty.
  11. Inability to Chew or Grind Certain Foods with the Fixed Prosthesis Teeth: Within the first six months, if the patient does not show contact points when biting on articulating paper, the repair and, if necessary, remaking of the fixed prosthesis are guaranteed.
  12. Speech Difficulties Due to the Fixed Prosthesis: Within the first 3 months after the fixed prosthesis is fitted, any speech difficulties caused by the fixed prosthesis will be addressed under the warranty.
  13. Issues with the Aesthetic Appearance and Color of the Fixed Prosthesis: Remaking of the fixed prosthesis or changes to the dental forms during the trial stages when the teeth are temporarily cemented or screwed in are covered by the warranty. However, any requests for aesthetic changes to the teeth after the fixed prosthesis treatment is completed are not included in the warranty.

Patients living abroad have an additional 3 months on top of the valid warranty periods, shorter than 1 year, if they can prove the issue to Citydent within the warranty period and cannot come to Citydent immediately. However, during this time, they must act according to the instructions of Citydent's doctors.


Conditions Outside the Warranty Coverage

  1. Problems with the Fixed Prosthesis and Parts: If any loosening, cracking, or breaking occurs in the prosthesis or parts for any reason, the patient must inform Citydent immediately and follow the dentist's instructions until a new prosthesis is made. Citydent is not responsible for implant losses and prosthesis problems that may occur as a result of usage contrary to the instructions.
  2. Infection: If any signs of infection are observed in the implant or prosthesis area, the patient must quickly inform Citydent and act according to the dentist's instructions. Possible outcomes are not covered by the warranty if the infection is not reported in time and instructions are not followed.
  3. Teeth Grinding Problem: Patients with a teeth grinding problem must follow the dentist's instructions and use the given mouthguard accordingly. Citydent is not responsible for implant losses or dental prosthetic issues that may arise if the patient does not follow these instructions.
  4. Oral and Dental Care: The patient must take care of their daily oral and dental hygiene after implant treatment. Citydent is not responsible for implant losses or dental prosthetic problems that may arise due to not maintaining regular and necessary oral and dental care.
  5. Gum Diseases and Related Issues: Citydent is not responsible for the loss of implants or potential problems with the prosthesis that may result from bone and gum loss due to gum diseases that may occur independently of the implant treatment after implant placement.
  6. Physical Traumas: Citydent is not responsible for damage to the jawbone, teeth, or prosthesis due to great forces such as falls, traffic accidents, and subsequent implant loss and potential problems with the prosthesis.
  7. Regular Dental Checkups: The patient is required to undergo dental check-ups at least once a year. Otherwise, the warranty program is not valid.
  8. Treatments Outside of Citydent: Citydent is not responsible for implant losses or prosthetic problems that may occur due to dental treatments performed outside of Citydent.

Other Provisions

Fees for Treatments Performed Outside of Citydent: Citydent is not responsible for any fees that may arise from implant treatments, dental prosthetic cementing, dental prosthetic repairs, or dental prosthetic renewals performed outside of Citydent for implants and overdentures made at Citydent. In these cases, no reimbursement will be made to the patient.

Multiple Remakes or Repairs of the Prosthesis: Within the warranty periods, the dental prosthesis can be remade or repaired multiple times. The warranty period for a prosthesis that is remade under warranty does not restart; the existing warranty period continues to apply. For patients coming from abroad, a refund of €250 will be made if they visit Citydent within the first year for issues that may occur within the first six months under this warranty program.
