How Long Is The Implant Treatment
Thanks to knowledge, skills, experience and technological opportunities, we are capable of replacing all of your teeth (requires in total 12-14 implants) and treating even toothless mouths in one day. In some cases, the surgical procedure can be divided into a few sessions, especially for the comfort of the patient. Our average time of surgery for a single dental implant is 15 minutes, though this duration differs from patient to patient. For an implant treatment, you will need 3 to 5 days for surgical phase including temporary prosthesis and control session.
After the surgical procedure, there is a waiting period which takes 2-3 months for the lower jaw and 3-5 months for the upper. During this waiting period, a prosthesis is placed on the standard implant. This interim period is necessary because we have to wait for a healthy bone tissue to appear between the implant and jawbone before further procedures. After waiting for a sufficient amount of time, the placed implants area is opened and measured for the prosthesis. The prosthesis stage is finished in 3-5 sessions and takes about a week.

SURGERY PHASE
Open surgery: Surgical placement of dental implants on toothless area after the relevant area or areas are anesthetized. The area of the procedure is made numb so that the implant can be inserted into the bone without pain. However, in some cases it is also possible to carry out the surgical procedure with general anesthesia.
Guided surgery:
Also known as ‘the without surgery method’. In this method, the dental tomographic image and in-mouth measurements of the patient are taken, and then, used to prepare a surgical guide of the area where the implant will be placed. This guide helps the surgeon to place the dental implant without cutting and removing the gums. This method does not require suturing because there are no scalpel-cut tooth areas.
Guided surgery method is more comfortable for patients. It shortens the surgery time. There is less bleeding and pain after surgery and no cutting or stitches at surgery. It offers a shorter recovery time for the patient.
Immediate Implant:
In many cases, it is possible to place a dental implant into the socket of the removed tooth right after the extraction. This method is similar to the closed (guided) surgery method in that it does not involve any flaps or surgical cuts. On the other hand, it is also similar to the open surgery method since it enables the surgeon to visually observe the bone area where the dental implant will be placed. This method usually does not require suturing. It is quite an advantageous method as it shortens the time required for the procedure and that it does not require a second operation. After the dental implant surgery, a temporary crown or a temporary denture is often put into the toothless area of the patient. Yet, the use of temporary dentures is not a requirement for all cases.
Bone Grafting:
Patient may need bone grafting before or during implant surgery if the jawbone is not thick enough or the quality of the bone is not good enough. That’s because the action of your mouth exerts great pressure on your bone, and if the jaw bone can not support the implant, the implant would probably fail. Thus, bone grafting can create a more solid environment for the implant.
In bone grafting, a piece of bone is removed from another part of your jaw or your body — your hip, for example — and transplanted to your jawbone. Another option is to use artificial bone (commercially available bone) to place in these areas. It may take several months for the transplanted bone to grow enough to support a dental implant. Quality of bone graft is very important for success of the surgery. We apply internationally well-known certified grafts in our clinic.
Sinus Lifting:
The upper back jaws are one of the most difficult areas to place dental implants due to insufficient bone quantity and quality and the close proximity to the sinus of the patient. If a patient has lost significant amount of bone in the area of upper back jaw due to reasons such as periodontal disease or tooth loss, sinus lifting might be necessary before the implant surgery. Sinus lifting operation can solve this problem by raising the sinus floor and applying bone for the placement of dental implants.
There are two different techniques for sinus lifting: open sinus lifting and closed sinus lifting. Which method to use will be chosen according to the amount of the related bone deficiency. Dentist will decide the method after examining patient’s X-ray and/or Tomography. It may be possible to place implants simultaneously at the same session, but in some cases, sinus lifting has to be done primarily and the graft is left to heal for several months before the implant procedure.

In terms of its structure, the implant is a screw made of titanium, which is placed inside the bone instead of the lost tooth. Implant treatment is the process of placing the implant in such a way that it acts as an artificial tooth root, instead of the tooth that was pulled out due to periodontal reasons, caries, trauma or other reasons. Bone loss are usually observed in the area where the teeth are missing over time. This, in turn, significantly reduces the comfort of patients using dental prosthesis alternatives, including implants. In some advanced cases, it may even come to a situation where the prosthesis cannot be used. Implant treatment is usually performed immediately after tooth extraction, without damaging the adjacent teeth (eg, without “trimming” the teeth as required for bridge treatment). In order to perform implant treatment, the bone in the area must be of appropriate height and width. The real state of the bone tissue can only be understood with 3D tomography.
Implant treatment is usually a two-stage treatment, and in some rare cases, a three-stage treatment. Depending on the condition of the bone, in cases where the implant cannot be placed immediately after tooth extraction, ossification of the relevant area is expected for at least 2-3 months. At the end of this period, a 3D image of the relevant region is taken and an implant with a diameter and size suitable for the bone is placed. After the implant is placed, it can be waited for 6 months (especially in advanced surgery cases with open sinus lifting, bone graftings and membrane use, etc.) for osseoentegration and to be able to carry the porcelain crown that will come on it. It can be said that there is a waiting period of 3 months for the upper jaw and 2 months for the lower jaw during the entire treatment. After the implant is fused to the bone, a porcelain tooth can be made by taking an intraoral measurement.

After tooth extraction, the hard tissue dissolves by an average of 0.8-1 mm each year, and if the area will remain toothless for many years, it’ll make it difficult to place an implant later. In some cases, patients may experience gingival recession and bone resorption due to gingival disease, or the use of removable prosthesis may cause the bone to dissolve faster. Bone graftings are a form of treatment that is frequently applied and successful results are obtained in patients with osteoporosis of the jaw. In cases that have been edentulous for many years, implant treatment can be performed using bone powder and membrane.
The sinuses are located in the spaces inside the bones on both sides of the nose. Anatomically, some of the roots of our molars are either adjacent to or within the sinus cavities. In cases where implants cannot be performed in the same session after the extraction of these teeth, the sinuses may droop towards the formed space while waiting for the bone to heal. In cases where the sinus cavities are too drooping to place implant in the bone, “bone powder” and membrane are used in that area in the sinus lifting operation. If there is enough bone, the implant is placed in the same session, but in some cases, it may be necessary to wait 6 months for the ossification of the sinus lifting area.
Implant companies give a lifetime warranty on implants. However, plaque, bacterial plaque, inflammation, etc. formed on our own teeth can also occur in the implant area and on your implant prosthesis. Once you have an implant, you should apply the care you make to your natural teeth (tooth brushing, using an interface brush or dental floss, using mouthwash) to the implant and prosthesis on the implant. When you perform oral hygiene regularly, you will use your implant for many years, but if you do not, bone loss may occur in the neck area of the implant. For this reason, you should not neglect to visit your dentist regularly every 6 months or once a year.
Implant treatments are performed under local anesthesia and are often a more atraumatic procedure than tooth extractions. After anesthesia, you will not have any pain during the operation. It is important that you use the prescription given to you in the direction directed by your physician for the future. Since a bone-related procedure is performed, swelling or mild pain is normal, but you should continue to use the medications you use regularly to keep your swelling and pain at a minimum level.
In seamless implant treatments, a 'guide mold' specially prepared for the patient is made by taking 3D tomography. By means of this guide, the implant size and diameter to be placed in the area where the patient's missing tooth is predetermined. A guide is created at the depth and width where the implant will be placed on the gingiva called punch. In this way, the implant is placed without lifting the flap (gingiva) and there is no need for stitches afterwards. In this way, there is not much bleeding in the surgical area and the operation takes a shorter time.
The prostheses that fixed to the implants like screws instead of cementing the prostheses on the implants are called screw retained prostheses. There are holes in the occlusions of the prostheses made on the implants in this system. These holes are filled with composite at the end of the treatment. Screw retained prostheses have many advantages over cemented (adhesive) prostheses. The biggest of these advantages is that they are not “glued” inside of the mouth. In case of any problem, it can be removed and repaired more easily than cemented prostheses. Patients cannot remove the prosthesis from the mouth by themselfs.
Fixed temporary prostheses are produced in order to ensure the patient's pronounciation and self-confidence rather than the eating function during the osseointegration stage. Implants should not be burdened during this process. For this reason, fixed temporary teeth can only be produced in tooth deficiencies in the anterior region or in complete edentulism. Temporary teeth placed on the implants are fixed and patients cannot remove it by themselfs. Fixed temporary teeth should be made within maximum 3-4 working days after the implants are made. Since fixed temporary teeth are made before the soft tissues are fully healed, prosthesis is made according to the post-surgical condition of the soft tissue. After the gingival healing, there may be gaps under the prosthesis. It is very important to be able to clean around the implant and under the prosthesis.
In the absence of a single tooth, an implant can be made in the relevant area. In the absence of two teeth next to each other, it is necessary to make two implants because the prosthesis to be made on it will not be able to carry a single implant since it will be for two teeth. However, it is not recommended to place an implant for every missing tooth. If the patient has too many missing teeth, three teeth can be made on two implants in the upper jaw. A maximum of four teeth can be made on two implants in the lower jaw. In fully edentulous mouths, a maximum of 6-8 implants can be made in the upper jaw and 12-14 teeth can be made. 12-14 teeth can be made with 6-8 implants in completely edentulous mouths in the lower jaw.
It is the right thing to do to consultate the physician who regularly monitors the patients with heart, diabetes or blood pressure or alike problems before the implant suegery. For example the surgery cannot be performed because anticoagulants prevent blood from clotting. Patients should stop the anticoagulant in consultation with their doctor.
We recommend that you decide on the brand of the implant in with your surgeon. One of the most important factors that will guide us in choosing the implant brand is the condition of the bone tissue. In patients who have been edentulous for many years, it may be necessary to use thinner implants as the jaw bone is thinner. Another criterion is that the implant brand you will choose has been on the market for many years and has proven its sustainability. It is important that the brand has a low or no failure rate in academic research, and that there is no bone loss in the neck area in the implants made 10-15 years ago - depending on the oral hygiene of the patient. Not only the implant but also the implant parts are very important in prosthesis production. One of the issues to consider when deciding on an implant brand is the availability of these parts and their suitability for digital dentistry.
Before the intraoral examination, our patients can send us photographs and x-ray images. In most cases, although this x-ray and intraoral photograph clarifies a certain part of the treatment, the situation we see inside the mouth may be slightly different. A tooth that we do not see a problem in the X-ray image may have broken, cracked or periodontal gum loss in the mouth. In such cases, the course of treatment may change. The price given in advance for the implant does not change, but in cases where extra bone grafts, membranes or other materials will be used in the area to be implanted after the tomography taken, these are added to the planning and information is given.

Zirconium-ceramic crowns, which provide a more aesthetic appearance and it’s better gingival compatibility are more preferred. Zirconium-ceramic crowns does not cause an allergic reaction and has a higher light transmittance. However, it is more expensive than the metal substructure coating. Metal-ceramic crowns, on the other hand, are more robust and has less fragility than zirconium-ceramic crowns, but less preferred. If the prosthetician is working with a laboratory that follows and applies the latest technology, gingival harmony and aesthetics can be achieved on metal-ceramic crowns like zirconium-ceramic crowns. Since the light transmittance is less, it is more common to use for functional purposes rather than aesthetic purposes. That changes according to teeth and bone conditions. So we can answer your questions more spesificially in terms of this topic only after Ct Scan.
Dental implants loves vertical forces, not horizontal ones, so it is necessary to have the crown in 1 year at most after the implant is placed. If the crown is not get, patient will lost some bone tissue in the implant area. After the implant is placed, regular attention should be paid to oral hygiene during the waiting period. In cases where oral hygiene is lacking, food residues and bacterial plaque accumulation are experienced in the implanted area. Such situations cause the implant to fail, that is, not to fuse with the bone. One of the other considerations is the recovery of soft tissue after surgery. Smoking affects this negatively. While there are still stitches to the surgical area in the mouth, smoking causes the closure of the soft tissue and nicotine accumulation on the sutures.
When the implant fails, tomography is taken to see how much bone loss is in the area and whether there is bone suitable for a new implant. If the bone volume is available, a new implant is made.
Making porcelain teeth on the implants in the week they are placed is called early loading. All logic in dental implants is based on osseointegration. Osseo means bone in Latin. Osseointegration means to integrate into the bone, to fuse. In order to make fixed functional teeth, we wait for the implant, which we receive support from the bone, to integrate into the bone. In early loading, prosthesis is placed on the implant(s) without waiting for the union of this bone. The most important criterion for early loading is the tightness of the implant as it settles into the bone. This is called primary stabilization. If there is a sufficient number of implants in cases of complete edentulism, early loading can be done by connecting these implants to each other. Mandibular removable / palate prostheses can be made more stable with early loading with 2 implants. Aesthetic problems in missing anterior teeth can be solved with early loading. In cases where early loading cannot be performed, fixed temporary teeth assist patients in the osseointegration process.
